Nominations Sought for UCSF Mount Zion Milton and Helen Pearl Award for Outstanding Service
Nominations are now being accepted for the 2009 UCSF Mount Zion Milton and Helen Pearl Award for Outstanding Service.
The purpose of the award is to recognize and reward a staff member who has provided service to the Mount Zion campus of UCSF, and has consistently performed at an extraordinary level, above and beyond normal job requirements.
The nominee must have demonstrated exceptional willingness to render additional service of consequence to others, such as patients, patients’ families, physicians, co-workers and others.
The award will be presented at this year’s UCSF Founders Day awards ceremony on May 1. The awardee will receive a certificate and $1,500 funded by the Pearl family.
The nominee must be a current UCSF employee with a minimum of two years of service to UCSF Mount Zion, including employees working in support of Mount Zion patients and staff at Parnassus, Laurel Heights, Mission Center Building or elsewhere.
The nominator must complete a nomination form, which requires the signatures of the nominator and two individuals who second the nomination and can attest to the nominee’s remarkable record of service. The nominator must also include a written description of the nominee’s outstanding service to UCSF Mount Zion.
Final nominations are due by noon on Friday, Feb. 27. Nomination forms should be sent to Nannette Diokno, Box 0914, or hand delivered to 350 Parnassus Ave., Suite 210.
