This signature must be witnessed by a University employee. An employee cannot work prior to signing this form in front of a University witness. In order to comply with this University policy and assure timely set up in UC's payroll system, all UCSF Stanford employees who become UCSF employees must complete this process by March 10, 2000. Employees who are on vacation or leave of absence during this period must sign the form prior to starting work at UC. To facilitate this process, over 70 UCSF employees have volunteered to be oath/patent witnesses. Medical Center departments have lists of these volunteers. If you have not done so already, see one of these oath/patent witness volunteers as soon as possible. The witnesses will return the forms to the UCSF Transition Office. Links: |
